PLEASE READ-REQUIRED INFORMATION!
ILLINOIS STATE LAW requires incoming freshmen, students transferring from another school OR home schooled students must present a current health physical examination and evidence of up-to-date immunizations. A completed physical examination, which must be dated within one year of the start of school and proof of immunizations must be submitted BEFORE a student can begin school in August. **SPORTS PHYSICALS ARE NOT ACCEPTED for this examination**
Below is the “Certificate of Child Health Examination Form ”. Both forms need to be completed, including the health history portion which must be filled out and signed by the parent/guardian, before the examination. This form will work as the “sports physical” if these students plan to take Interscholastic Sports. Please use the below check-list to ensure all proper steps have been taken.
Vision examinations are REQUIRED for out of state and home schooled students & Dental examinations are recommended at this time. * Dental Form
Students planning to participate in the athletic program (Sophomore, Junior & Seniors) require a sports physical. This does not apply to incoming freshman because they need a “Certificate of Child Health Examination Form”, which will work as the sports physical. We encourage you to schedule examinations in June, before sports practice begins in mid-August, for students to be able to participate for the entire school year. * Sports Physical Form
** NEW** Before a student can participate in athletics, he/she MUST obtain an ELIGIBILITY SLIP from the AD office and give it to their coach. This slip is mandatory for an athlete to turn into his/her coach, or they will be not able to try out.
IMMUNIZATIONS- The immunization history shall include the dates (MM/DD/YY) that the vaccinations were administered and must be signed and dated by a physician or nurse verifying that they were given.

REMEMBER THAT THE DEADLINE FOR PROVIDING THE SCHOOL WITH THE NECESSARY PHYSICAL AND IMMUNIZATIONS FOR STUDENTS IS THE FIRST DAY OF SCHOOL. YOUR CHILD MAY BE EXCLUDED FROM SCHOOL WITHOUT THIS PAPERWORK.

FOR STUDENTS WITH MEDICAL CONDITIONS, please work with you medical provider to complete the appropriate forms and return to the school nurse. Providing this information will help with managing the condition while the student is at school and will also serve as the medical order for any medication required during the school day.
